Make a referral
How to refer someone for an independent assessment, what we need from you, and what happens next.
Who can refer
Capacity, best interests and risk assessment referrals are accepted now. Referrals for ADHD and autism assessment are not open yet.
Local authorities and care providers
Social work teams, DoLS teams, care homes and supported living services instructing an independent assessment.
Solicitors
Court of Protection work, property and affairs matters, and cases where capacity is disputed.
Health and social care professionals
GPs, nurses, social workers and allied professionals, with the person’s consent or another lawful basis.
Individuals and families
You can contact us directly to discuss a capacity assessment. Call us and we will talk it through before any paperwork.
What happens after you refer

We acknowledge it
We confirm we have received the referral and tell you whether we can take it, usually within one working day.

We check it is the right assessment
A short conversation about the specific decision in question. Getting this wrong at the start wastes everyone’s time.

We arrange the appointment
At our premises, at the person’s home, or at their placement, depending on what suits them.

You receive the report
A written report with the reasoning set out, not just the conclusion.
What we need from you
A referral moves faster when these are included. If you do not have all of it, send what you have and we will ask about the rest.

Complete the form and we will contact you to take the clinical detail securely. Please do not enter medical history, diagnoses or other health information in this form.
Why the form does not ask for clinical detail
A referral contains special category health data about someone who is often not able to consent to how it is handled. Until secure submission is connected, we take that detail by telephone or through an agreed secure route rather than through a web form.

Not a crisis service
